      GERIATRIC NURSE PROGRAM


A Geriatric Mental Health Nurse is a registered nurse with extensive experience and expertise in dealing with mentally impaired elderly individuals.

She provides a link for seriously and persistently mentally ill adults to the appropriate medical and social services and resources within the community.

The goal of the Geriatric Community Mental Health Nurse Program is to provide clients with the appropriate services and treatment that will ultimately delay or prevent hospitalization.  This is accomplished through assessment and evaluation of the client's needs.  The nurse also works closely with the client's family and other professionals and agencies to design an appropriate treatment plan.

Individuals who are 50 years of age and older who are homebound and have a history of serious and chronic mental illness are eligible for the program.
